Verify Your Insurance Benefits
We are in network providers with BCBS PPO plans (including Blue Choice) and select UnitedHealthcare plans. Coverage for nutrition counseling varies by plan. Please contact your insurance carrier by calling the number on the back of your card or messaging them in the insurance portal to confirm that your plan includes nutrition counseling and understand any out of pocket costs you may incur.
Prior to our first session, please contact your insurance carrier to verify your benefits. Below are questions to ask your insurance carrier to help determine your coverage.
Does my plan cover the following outpatient nutrition counseling, procedure codes 97802 and 97803? If you are asked for a diagnosis code please provide Z71.3
Do I need a referral or prior authorization?
Do I have a co-pay for each visit? If so, how much is my co-pay?
Does my deductible apply? If so, how much is my deductible? How much of my deductible has been met so far this year?
Do I have co-insurance? If so, what percentage am I responsible for?
Do I have any session limits or max? If so, how many sessions do I have remaining? What is my benefit period?
Is Be Well Nutrition Counseling, NPI number 1891401089, an in-network provider with my plan? Practice address 730 E Thacker Street, Des Plaines, IL 60016.
If not, what are my out-of-network benefits and what is the process to submit for reimbursement?

If you have a different plan or carrier, you may be able to use your out-of-network benefits for our sessions. We can provide you superbills which you will be able to submit to your insurance for direct reimbursement if your plan includes out-of-network coverage for nutrition services. Please contact us to discuss self pay rates by phone or email. We accept credit cards, debit cards, and FSA/HSA cards. If you are not using your insurance benefits, payment is due at time of service.
