Verify Your Insurance Benefits

Ana is an in network provider with BCBS PPO plans and select UnitedHealthcare plans. However, each plan is unique and we recommend you contact your insurance carrier by calling the number on the back of your card or messaging them in the insurance portal to confirm that your plan includes nutrition counseling and understand any out of pocket costs you may incur.

Prior to our first session, please contact your insurance carrier to verify your benefits. Document the name of the representative and the reference number for the call along with your benefits report.

You will need the following information:

1. Your name and date of birth

2. Your insurance Member ID Number

3. Your insurance Group Number

Below are questions to ask your insurance carrier to help determine your coverage.

1. Is Ana Pruteanu, NPI number 1649699778, an in-network provider with my plan? Practice address 730 E Thacker Street, Des Plaines, IL 60016.

2. If not, what are my out-of-network benefits?

3. Does my plan cover the following outpatient nutrition counseling, specifically CPT codes 97802 (Medical Nutrition Therapy Assessment) 97803 (Medical Nutrition Therapy Follow-Up) and S9470 (Nutritional counseling, dietitian visit)?

4. Are nutrition counseling benefits covered under preventative or medical coverage? If you are asked for a diagnosis code please provide Z71.3 or ask the representative which diagnosis codes are covered under your plan.

5. Do I need a referral or prior authorization?

6. Do I have a co-pay for each visit? If so, how much is my co-pay?

7. Does my deductible apply? If so, how much is my deductible? How much of my deductible has been met so far this year?

8. Do I have co-insurance? If so, what percentage am I responsible for?

9. Do I have any session limits or max? If so, how many sessions do I have remaining?

10. What is my benefit period?
