How To Verify Your Benefits

Prior to our first appointment, please contact your insurance carrier to verify your plan’s coverage and understand out of pocket costs you may incur. 

Below are questions to ask your insurance carrier to help determine your coverage. 

  1. Is Be Well Nutrition Counseling, NPI number 1891401089, an in-network provider with my plan? Practice address 730 E Thacker Street, Des Plaines, IL 60016.
  2. If not, what are my out-of-network benefits and what is the process to submit for reimbursement?
  3. Does my plan cover the following outpatient nutrition counseling, procedure codes 97802 (medical nutrition therapy assessment) and 97803 (medical nutrition therapy follow up)?
    • If you are asked for a diagnosis code please provide Z71.3 or F50.9
  4. Do I need a referral or prior authorization?
  5. Do I have a co-pay for each visit? If so, how much is my co-pay?
  6. Does my deductible apply? If so, how much is my deductible? How much of my deductible has been met so far this year?
  7. Do I have co-insurance? If so, what percentage am I responsible for?
  8. Do I have any session limits or max? If so, how many sessions do I have remaining? What is my benefit period?
  9. Please record the following:
    • Name of representative
    • Reference number for the call
    • Date of the call
